Dear Participant:

The Trustees have elected to change the Prescription Benefit Manager (“PBM”). Effective February 1, 2013, the current PBM, OptumRx, will be replaced by Citizens Rx. Please note that there was no change made to the Plan design, which means that the co-payment structure will remain the same.

Please be sure to check whether the current retail pharmacy you and your eligible dependents are utilizing is a participating provider. Also, if you have a mail-order prescription, please contact Citizens Rx for a claim form and the mailing address to mail the completed form. Additionally, references within the Summary Plan Description (SPD) appearing on pages 2, 4, 45, 46, 48, 50, 63 and 65 and elsewhere in the SPD to Optum Rx are changing to Citizens Rx effective February 1, 2013.

This Notice is a Summary of Material Modifications ("SMM") within the meaning of Section 104 of the Employee Retirement Income Security Act of 1974. An SMM describes changes to the information provided in the most recent SPD. The SMM describes important changes to the Plan effective as of the date listed above. Please keep this SMM with your SPD for future reference. Please contact the Fund Office if you would like to request a copy of the SPD or any SMM relating to the Plan.
