Erroneous Mailing of ID cards by Anthem

Please note there was a recent change in the vendor providing Life and Accidental Death & Dismemberment benefits for Active Participants in t he Plan from MetLife to Anthem Blue Cross effective October 1, 2013. During the course of these changes, Anthem Blue Cross erroneously issued ID cards to all participants enrolled for Life and Accidental Death and Dismemberment benefits.

We would like to assure you that there has been no change to the medical health plan you selected and enrolled in.

The Administrative Trust Funds Office would like to apologize for any confusion or inconvenience caused due to the recent mailing of ID cards from Anthem Blue Cross.
